Leon ISD Health Services Consent

What a great start to our school year at Leon ISD!  We are excited to see all of our students thriving as the year begins.

Texas Senate Bill 12 issued a new requirement for all parents to release consent for our nurses and counselors to be able to conduct any scope of physical or emotional services with our students. This includes services as routine as applying a band aid or ice pack to a minor injury.

In order for your child to be able to receive nursing or counseling services, you will need to return the attached documents with your signature.  There are TWO signature pages, one for counselors and one for nurses.  A paper copy will be sent home today (Tuesday folders for elementary students) that will need to be returned as soon as possible for your child to receive these types of services.  You may also scan a signed copy and email it to:

Elementary school:
